New DirectoryIndex and mime type entry

We found out that many people use index.htm as index file. I've added this entry to DirectoryIndex srm.conf at japan.park.org. It now looks like:

DirectoryIndex index.cgi index.html index.htm index.shtml

I've also added a new mime type for MIDI data file:

audio/x-midi mid midi

to mime.types.
